Foursquare Healthcare Data Breach Investigation

Foursquare Healthcare is short-term rehabilitation and long-term nursing care facility in Texas. As a rehabilitation and nursing care facility, Foursquare Healthcare provides its patients with rehabilitation, skilled nursing care, assisted living, and independent living services. Founded in 1978, Foursquare Healthcare prides itself on offering these services with a healthy dose of Texas hospitality.2 Since its founding, Foursquare Healthcare has grown to a system of 10 facilities throughout the state of Texas. Headquartered in Rockwall, Texas, Foursquare Health employs over 1,000 individuals.


Recently, Foursquare Healthcare discovered that it had experienced a data breach in which the sensitive personal identifiable information and protected health information in its systems may have been accessed. Through its investigation, Foursquare Healthcare determined that an unauthorized actor may have accessed this sensitive information. In November 2023, Foursquare Healthcare reported the data breach to the Attorney General of Texas. In its report, Foursquare Healthcare reported that it has not yet begun to notify individuals whose data may have been impacted. The type of information exposed includes:

  • Name
  • Social Security number
  • Address
  • Date of birth
  • Driver’s license number
  • Financial account information
  • Medical information
  • Health insurance information
